Mood Media Employee Lead Referral Program

Welcome to the Lead Referral Program! We applaud your initiative to collect and share a sales lead, and your participation shows your daily dedication to the BASICS. With each lead you enter through this form, you play an active role in achieving Mood Growth and (just as importantly) earn chances at winning $50. It's easy to enter - just fill out as much of the information on the right as possible and then click Submit. Please note that some information is mandatory.

HOW IT WORKS
If your submitted lead results in a signed agreement, we automatically give you a $50 gift card! And if your lead doesn't pan out, is a duplicate lead or is already a client, you will still have a chance at the prize - every quarter, we will gather all of the entered leads together and randomly select two names to win $50 gift cards. Remember:

• There is no limit to the number of leads you can enter, or the number of $50 gift cards you can win. The more you enter, the better your chances of winning!
• Leads need time to turn into sales, and sometimes it may take several months. Just be patient. If your lead is in IA territory, the lead must be audited and may take longer to process.
• If your lead has already been submitted by another employee, you will not be eligible for $50 if it turns into a sale. However, you will still be entered into the quarterly drawing.
• Account Executives, Business Development Representatives and Sales Management are not eligible to participate in this program.
